Acknowledgment of Risk

Participation in basketball and related athletic activities involves inherent risks, including but not limited to falls, collisions, physical contact, equipment failure, head injuries, concussions, serious bodily injury, and other risks associated with athletic participation.

By registering, I acknowledge that I am the parent or legal guardian of the participant and understand that participation in basketball activities may involve risks that cannot be completely eliminated.
